Oritavancin is a semisynthetic lipoglycopeptide antibiotic used for the treatment of acute bacterial skin and skin structure infections (ABSSSI) caused by susceptible Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococci (VRE), and streptococci. It is administered intravenously, typically as a single dose due to its long half-life. Oritavancin acts through multiple mechanisms: it inhibits transglycosylation and transpeptidation steps in bacterial cell wall synthesis by binding to peptidoglycan precursors and pentaglycyl bridging segments, and it disrupts the integrity of the bacterial cell membrane, leading to rapid bactericidal activity. Its extended tissue penetration makes it suitable for outpatient therapy in difficult-to-treat populations[1][2][6][7].
