OSI-7904L + cisplatin is a combination regimen of two anticancer agents used in clinical development for advanced solid tumors, notably gastric and breast cancer. OSI-7904L is a liposomal formulation of a potent, noncompetitive thymidylate synthase inhibitor that does not require polyglutamation for activity. Its encapsulation in liposomes enables prolonged plasma residence and potentially enhanced tumor localization. Cisplatin is a well-established DNA-platinating agent that causes crosslinking of DNA, resulting in apoptosis of rapidly dividing cancer cells. The combination is designed to exploit synergy between DNA damage caused by cisplatin and inhibition of nucleotide synthesis by OSI-7904L. Primary clinical investigation has focused on intravenous administration, with the recommended phase II regimen being 75 mg/m² cisplatin over 2 hours followed by 7.5 mg/m² OSI-7904L over 30 minutes every 3 weeks. The regimen has shown preliminary activity in gastric and breast cancer with manageable toxicity, and no major pharmacokinetic interactions between the agents have been observed[1][3][5].
