Overview

Osimertinib + ramucirumab is a combination therapy under investigation for the treatment of advanced non-small cell lung cancer (NSCLC) with epidermal growth factor receptor (EGFR) mutations. Osimertinib is a third-generation, irreversible EGFR tyrosine kinase inhibitor that selectively targets both EGFR-sensitizing and T790M resistance mutations. Ramucirumab is a human IgG1 monoclonal antibody that inhibits angiogenesis by binding to vascular endothelial growth factor receptor 2 (VEGFR2), blocking its activation by VEGF ligands. The rationale for combining these agents is to simultaneously inhibit tumor cell proliferation via EGFR blockade and suppress tumor angiogenesis via VEGF pathway inhibition, potentially delaying resistance mechanisms and improving clinical outcomes. Clinical trials have shown this combination significantly prolongs progression-free survival compared to osimertinib alone in patients with TKI-naïve EGFR-mutant NSCLC, with an acceptable safety profile[1][2][3][6].
