Osteoprotegerin-Fc (OPG-Fc) is a recombinant fusion protein consisting of the ligand-binding domain of human osteoprotegerin (OPG) fused to the Fc portion of human IgG1. It functions as a decoy receptor for the Receptor Activator of Nuclear Factor kappa-B Ligand (RANKL), a key cytokine involved in osteoclast differentiation and activation. By binding to RANKL, OPG-Fc prevents it from interacting with its receptor, RANK, on osteoclast precursors, thereby inhibiting bone resorption and osteolysis. Developed by Amgen (as AMGN-0007), it was investigated for the treatment of osteoporosis and bone metastases associated with various cancers, including non-small cell lung cancer (NSCLC). Although it showed clinical activity in early-phase trials, its development was halted by Amgen in favor of denosumab, a fully human monoclonal antibody against RANKL that offered superior pharmacokinetic properties and a lower risk of inducing neutralizing antibodies against endogenous OPG.
