OT-1096 is a first-in-class small molecule immunomodulator being developed by Oblique Therapeutics. It targets the thioredoxin reductase (TrxR)/thioredoxin (Trx) axis, a critical component of the cellular redox system. The drug exerts a dual anti-tumor effect: it directly induces cancer cell death by converting TrxR into pro-apoptotic SecTRAPs (selenium-compromised thioredoxin reductase-derived apoptotic proteins) and provides immunomodulatory effects by downregulating regulatory T cells (Tregs) within the tumor microenvironment. Primarily being developed for advanced metastatic triple-negative breast cancer (TNBC), OT-1096 has demonstrated significant tumor growth inhibition in preclinical humanized mouse models, outperforming pembrolizumab in some settings while maintaining a favorable safety profile.
