Otaplimastat (SP-8203) is a first-in-class small molecule neuroprotectant developed by Shin Poong Pharmaceutical for the treatment of acute ischemic stroke. It is a quinazoline derivative that acts as an inhibitor of matrix metalloproteinases (MMPs), thereby reducing blood-brain barrier disruption and brain injury associated with ischemic stroke and thrombolytic therapy. Otaplimastat also exhibits neuroprotective effects through anti-inflammatory and antioxidant mechanisms, including inhibition of reactive oxygen species and stimulation of superoxide dismutase activity. Clinical studies have shown that when used in combination with standard thrombolytic therapy (such as tissue plasminogen activator), otaplimastat improves functional outcomes, reduces infarct size, and lowers the risk of hemorrhagic transformation in patients with moderate to severe acute ischemic stroke[2][6][7][8][9].
