ATB-346 (otenaproxesul) is a hydrogen sulfide-releasing non-steroidal anti-inflammatory drug (NSAID) and a derivative of naproxen. It acts primarily by inhibiting cyclooxygenase enzymes COX-1 and COX-2, thereby suppressing prostaglandin production and exerting anti-inflammatory, analgesic, antinociceptive, immunomodulatory, and neuroprotective effects. Unlike traditional NSAIDs such as naproxen, ATB-346 is designed to reduce gastrointestinal toxicity by releasing hydrogen sulfide (H₂S), which has protective effects on the gastrointestinal tract. Preclinical studies have shown that ATB-346 inhibits inflammation without causing gastric damage typical of conventional NSAIDs. It has demonstrated efficacy in models of arthritis, traumatic brain injury (TBI), melanoma cell proliferation inhibition, and periodontal disease prevention. The drug is being developed by Antibe Therapeutics for indications including pain management and inflammatory conditions[1][3][5][6][7][8].
