Otilonium (commonly used as the bromide salt, otilonium bromide) is a small molecule antispasmodic drug primarily indicated for the treatment of irritable bowel syndrome (IBS) and other functional gastrointestinal disorders. It acts locally in the gut due to minimal systemic absorption and accumulates in the colonic wall. Otilonium exerts its effects by blocking voltage-dependent calcium channels (mainly L-type and also T-type), interfering with both intra- and extracellular calcium movement necessary for smooth muscle contraction. Additionally, it binds to muscarinic receptors and tachykinin neurokinin-2 receptors on smooth muscle cells and primary afferent neurons. These combined actions reduce abnormal gut motility, abdominal spasms, pain, bloating, and discomfort associated with IBS[1][3][4][5][6].
