Obgram (otilonium bromide) is a quaternary ammonium derivative that acts as a musculotropic antispasmodic. Marketed by Ingram Biosciences, it is primarily indicated for the symptomatic treatment of irritable bowel syndrome (IBS) and other gastrointestinal conditions characterized by smooth muscle spasms. The drug's mechanism of action is multi-faceted, involving the blockade of L-type and T-type calcium channels, antagonism of muscarinic receptors, and inhibition of neurokinin-2 (NK2) receptors. This combined activity reduces intestinal hypermotility and visceral hypersensitivity.
