Overview

OTR4132 is a synthetic heparan sulfate glycosaminoglycan mimetic and member of the RGTA (ReGeneraTing Agents) family. It is a dextran-based biopolymer designed to protect and stabilize endogenous heparin-binding growth factors from proteolytic degradation, thereby enabling tissue repair and neuroprotection. Preclinical studies demonstrate that OTR4132 exerts neuroprotective effects in models of central nervous system injury, attenuating cholinergic neuron degeneration after partial immunotoxic lesions and promoting functional recovery. OTR4132 can cross the blood-brain barrier and accumulate in lesion sites, where it restores the extracellular matrix, enhances brain plasticity, and supports processes like neurogenesis and angiogenesis. Currently, OTR4132 is in clinical evaluation for acute ischemic stroke following thrombectomy, with phase 2 trials underway to confirm efficacy and safety.
