OTT166 (formerly OQL061) is a potent, selective, small-molecule integrin inhibitor developed by OcuTerra Therapeutics for the treatment of diabetic eye diseases. Formulated as a topical eye drop, OTT166 is designed to reach the retina in therapeutic concentrations, offering a non-invasive alternative to the standard-of-care intravitreal injections. The drug acts as an antagonist to the integrins αvβ3, αvβ5, and αvβ6, which are key mediators of vascular permeability and pathological angiogenesis in the eye. By inhibiting these pathways, OTT166 aims to prevent the progression of diabetic retinopathy and reduce retinal thickening in patients with diabetic macular edema. The compound was evaluated in the Phase 2 DRIVE-DR clinical trial, where it demonstrated a favorable safety profile and evidence of biological activity, although it did not meet the primary efficacy endpoint in the overall study population.
