OX40L CAR regulatory T-cells (CAR Tregs) are an investigational cell therapy designed to treat chronic graft-versus-host disease (cGVHD) and other inflammatory conditions driven by antigen-presenting cells (APCs). These cells are engineered to express a chimeric antigen receptor (CAR) that targets OX40 ligand (OX40L), a co-stimulatory molecule selectively upregulated on activated APCs. By engaging OX40L, these CAR Tregs suppress pathogenic T cell activation through multiple mechanisms, including direct cytotoxicity against OX40L+ cells, trogocytosis (reducing surface OX40L expression), and modulation of APC activation. Preclinical studies in murine models of cGVHD and bronchiolitis obliterans have demonstrated that OX40L CAR Tregs can restore immune homeostasis and significantly improve pulmonary function.
