OX40L-FP (also known as MEDI6383) is a recombinant, hexameric fusion protein consisting of the extracellular receptor-binding domain of human OX40 ligand (OX40L) fused to an isoleucine zipper trimerization domain (derived from TRAF2) and a human IgG4P Fc domain. Developed by AgonOx and MedImmune (AstraZeneca), OX40L-FP acts as a potent agonist of the OX40 receptor (CD134, TNFRSF4) expressed on activated CD4+ and CD8+ T cells. By mimicking the natural ligand, it costimulates T-cell activation, proliferation, and survival, while reversing the immunosuppressive activity of regulatory T cells (Tregs). Although evaluated in Phase I clinical trials for advanced solid tumors, its clinical development was terminated by AstraZeneca in 2016.
