Overview

Oxaliplatin is a third-generation platinum-based chemotherapy drug classified as an alkylating agent. It is primarily used in combination with other agents (such as fluorouracil and leucovorin) for the treatment of advanced colorectal cancer and stage III colon cancer after surgical resection[4][5][7]. Oxaliplatin exerts its antineoplastic effects by forming both interstrand and intrastrand crosslinks with DNA, particularly at the N7 position of guanine residues. These crosslinks inhibit DNA replication and transcription, leading to cell cycle arrest and apoptosis[3][6][8]. Unlike earlier platinum drugs such as cisplatin or carboplatin, oxaliplatin contains a diaminocyclohexane (DACH) carrier ligand that contributes to its unique activity profile and reduced nephrotoxicity/ototoxicity compared to its predecessors[3][8]. The drug was first approved by the FDA in 2002 for use in colorectal cancer therapy[4].
