This is a multi-drug combination regimen consisting of six agents: - **Oxaliplatin** (a platinum-based chemotherapeutic agent that forms DNA crosslinks, inhibiting DNA replication and transcription) - **S-1** (an oral fluoropyrimidine derivative composed of tegafur [a prodrug of 5-fluorouracil], gimeracil [a dihydropyrimidine dehydrogenase inhibitor], and oteracil [which reduces gastrointestinal toxicity]; it acts as an antimetabolite interfering with DNA synthesis)[6][8][10] - **Amoxicillin** (a broad-spectrum beta-lactam antibiotic that inhibits bacterial cell wall synthesis) - **Clarithromycin** (a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit) - **Omeprazole** (a proton pump inhibitor that suppresses gastric acid secretion by inhibiting the H+/K+ ATPase in gastric parietal cells) - **Colloidal bismuth subcitrate** (forms a protective coating on ulcers, has antimicrobial activity against Helicobacter pylori, and may protect the ulcer base)[9] This combination brings together two regimens: 1. Oxaliplatin plus S-1 ("SOX")—used as chemotherapy for advanced gastric or colorectal cancer[2][4][5][8]. 2. Amoxicillin, clarithromycin, omeprazole, and colloidal bismuth subcitrate—a quadruple therapy for eradication of Helicobacter pylori infection[7][9]. The rationale for combining these regimens could be to treat patients with advanced gastrointestinal cancers who also require H. pylori eradication or ulcer protection during chemotherapy.
