Overview

This is a four-drug combination regimen consisting of oxaliplatin (a platinum-based chemotherapy), capecitabine (an oral prodrug of 5-fluorouracil), bevacizumab (a monoclonal antibody targeting vascular endothelial growth factor A), and a programmed death-1 (PD-1) antibody such as pembrolizumab or serplulimab. Oxaliplatin and capecitabine act as cytotoxic agents that inhibit DNA synthesis and cell division in rapidly dividing tumor cells. Bevacizumab inhibits angiogenesis by blocking VEGF-A, thereby reducing blood supply to tumors. The addition of a PD-1 antibody provides immune checkpoint inhibition, enhancing the anti-tumor immune response by preventing tumor-induced T-cell inactivation. This combination is under investigation primarily for metastatic colorectal cancer, especially in patients with proficient mismatch repair/microsatellite stable disease[2][4]. The regimen aims to combine direct cytotoxicity with antiangiogenic and immunotherapeutic effects.
