This treatment regimen, evaluated by UNICANCER, involves the locoregional delivery of oxaliplatin via hepatic arterial infusion (HAI) or isolated perfusion in combination with systemic (intravenous) leucovorin and 5-fluorouracil (5-FU). It is primarily developed for patients with unresectable colorectal liver metastases. Oxaliplatin is a platinum-based chemotherapy agent that forms DNA crosslinks, thereby inhibiting DNA replication and transcription. 5-fluorouracil is a fluoropyrimidine antimetabolite that inhibits thymidylate synthase, an effect that is biochemically potentiated by the addition of leucovorin (folinic acid). By delivering oxaliplatin directly into the hepatic artery, the regimen achieves high local concentrations within liver tumors while minimizing systemic toxicity, while the intravenous administration of 5-FU/leucovorin addresses potential extrahepatic disease.
