HAIC + TAE sequential therapy combined with tislelizumab and surufatinib is an investigational multi-modal treatment regimen for unresectable intrahepatic cholangiocarcinoma (iCCA). The regimen, currently being evaluated in the Phase II REACH-01 trial (NCT06239532) by Qilu Hospital of Shandong University, combines hepatic artery infusion chemotherapy (HAIC) using oxaliplatin and raltitrexed with transcatheter arterial embolization (TAE). This locoregional approach is supplemented by systemic therapy consisting of tislelizumab, a humanized IgG4 anti-PD-1 monoclonal antibody, and surufatinib, a small molecule inhibitor of VEGFR 1/2/3, FGFR1, and CSF-1R. The combination aims to leverage the synergistic effects of local tumor destruction, anti-angiogenesis, and immune checkpoint inhibition to improve outcomes in patients with advanced biliary tract cancers.
