Oxybutynin + clonidine is a combination of two small molecule drugs with distinct mechanisms of action. Oxybutynin is an antimuscarinic agent that acts as a competitive antagonist at postganglionic muscarinic receptors (primarily M1 and M3 subtypes) on the detrusor muscle of the bladder, resulting in relaxation of smooth muscle and reduction in symptoms associated with overactive bladder such as urinary urgency and frequency[2][6][8]. Clonidine is primarily an alpha-2 adrenergic receptor agonist that exerts central hypotensive effects by reducing sympathetic outflow from the central nervous system; it also has sedative properties and can decrease pain signal transmission[4]. The combination has been studied for its effect on reducing saliva production (sialorrhea), showing additive or synergistic effects in decreasing salivary flow without significant adverse events[5]. Each drug retains its primary indications—oxybutynin for overactive bladder and neurogenic bladder dysfunction, clonidine for hypertension, ADHD, and other off-label uses—but together they may be used experimentally or investigationally to manage sialorrhea.
