Oxybutynin, tolterodine, and solifenacin are a group of small molecule antimuscarinic (anticholinergic) drugs used for the treatment of overactive bladder (OAB) syndrome. They act as antagonists at muscarinic acetylcholine receptors, particularly the M2 and M3 subtypes located on the detrusor muscle of the bladder, thereby reducing involuntary bladder contractions and symptoms of urinary urgency, frequency, and incontinence. In clinical research, such as the Phase 4 trial sponsored by China Medical University Hospital, these agents are collectively used as a standard-of-care comparator arm to evaluate the efficacy and tolerability of newer therapies like beta-3 adrenergic agonists (e.g., mirabegron). This comparison is particularly relevant in patients with Sjogren's syndrome, where the systemic anticholinergic side effects of these drugs, such as dry mouth and constipation, can exacerbate the underlying symptoms of the disease.
