Overview

Oxybutynin + trazodone is an experimental pharmacological combination being investigated for the treatment of obstructive sleep apnea (OSA). This combination therapy targets two distinct endotypic traits of OSA: low upper airway muscle responsiveness and a low respiratory arousal threshold. Oxybutynin, a synthetic muscarinic antagonist, is intended to enhance the activity of the genioglossus muscle (the primary tongue dilator) to maintain airway patency during sleep. Trazodone, a sedative antidepressant and serotonin antagonist and reuptake inhibitor (SARI), is used to increase the respiratory arousal threshold, thereby reducing sleep fragmentation and preventing premature awakenings that can destabilize breathing. This approach aims to provide a non-mechanical, oral alternative to continuous positive airway pressure (CPAP) therapy. Research into this combination and related multi-drug regimens is being conducted by institutions such as China Medical University Hospital.
