Oxycodone DETERx is an extended-release, abuse-deterrent formulation of oxycodone designed to retain its extended-release properties even after physical manipulation such as crushing or chewing. It's marketed under the brand name Xtampza ER and uses a novel microsphere-in-capsule technology. The formulation consists of oxycodone base combined with fatty acids and waxes to form hydrophobic, waxy microspheres. This unique composition makes it difficult to extract the active ingredient for abuse while allowing alternative administration methods for patients who have difficulty swallowing. ## Pharmacokinetics and Abuse Deterrence Oxycodone DETERx maintains its extended-release properties even when manipulated. Studies have shown that chewed and intact oxycodone DETERx are bioequivalent, unlike crushed immediate-release oxycodone which yields higher peak plasma concentrations[1][6]. This is significant because: 1. The maximum effect (Emax) for drug liking was significantly lower for chewed and intact oxycodone DETERx compared to crushed immediate-release oxycodone[1][6] 2. The time to Emax was significantly longer for oxycodone DETERx formulations[1][6] 3. Scores for "feeling high" were lower for oxycodone DETERx compared to immediate-release oxycodone[1][6] These properties make oxycodone DETERx less attractive for abuse via oral routes, which often serve as a gateway to more dangerous routes of administration. ## Clinical Applications Oxycodone DETERx is indicated for the management of pain severe enough to require daily, around-the-clock opioid treatment for an extended period, when alternative treatment options are inadequate[3][8]. In clinical trials involving patients with chronic lower back pain, oxycodone DETERx demonstrated significant pain reduction compared to placebo[3]. A key advantage of this formulation is its versatility in administration. Unlike most extended-release opioids, oxycodone DETERx can be: - Administered by sprinkling onto soft food - Delivered via gastrostomy tube - Delivered via nasogastric tube[1] This provides important options for patients with difficulty swallowing or those requiring alternative administration routes.
