Oxymetholone is a synthetic anabolic-androgenic steroid (AAS) and a derivative of dihydrotestosterone. It is primarily indicated for the treatment of anemias caused by deficient red cell production, including acquired and congenital aplastic anemia, myelofibrosis, and hypoplastic anemias due to myelotoxic drugs. Oxymetholone acts mainly by stimulating erythropoiesis through enhancement of erythropoietin production in bone marrow failure syndromes[2][7][8]. Its mechanism involves activation of the androgen receptor, leading to increased protein synthesis and muscle mass. While it was once used for osteoporosis and HIV/AIDS wasting syndrome due to its anabolic effects[5], its use has declined because safer alternatives are available. The drug is known for significant side effects including hepatotoxicity[5][8].
