Oxyntomodulin is a naturally occurring 37-amino acid peptide hormone secreted by enteroendocrine L-cells of the gut in response to food intake. It acts as a dual agonist at both the glucagon-like peptide-1 receptor (GLP-1R) and the glucagon receptor (GCGR), combining effects of GLP-1 and glucagon. This dual action results in reduced appetite, increased satiety, enhanced energy expenditure, and weight loss. Oxyntomodulin has been shown to lower body weight more effectively than selective GLP-1R agonists alone and can improve glucose tolerance in patients with type 2 diabetes mellitus by counterbalancing the hyperglycemic effect of GCGR activation with GLP-1R-mediated glucose lowering. Its mechanism involves central effects on appetite regulation via hypothalamic pathways as well as peripheral metabolic actions. Clinical studies are ongoing for its use in obesity and diabetes.
