P-12 peptide-based immuno-nano-conjugates (P12/AP-2/NCs) are an innovative immunotherapeutic nanoplatform designed for targeted drug delivery across the blood-brain barrier (BBB) to treat central nervous system malignancies, specifically glioblastoma multiforme (GBM) and melanoma brain metastases (MBM). The conjugate utilizes a biodegradable, functionalized polymalic acid (PMLA) polymer backbone. It is conjugated with two primary functional peptides: P-12 (Aurigene NP-12), a small-size antagonist peptide that blocks the PD-L1 and PD-L2 immune checkpoints, and Angiopep-2 (AP-2), a ligand for the LRP-1 receptor that facilitates BBB penetration via receptor-mediated transcytosis. By delivering these checkpoint-blocking peptides directly to the tumor immune microenvironment (TiME), the nanoconjugate aims to reinvigorate T cell proliferation and overcome immune evasion. Research indicates that this platform provides superior brain accumulation and a more favorable safety profile compared to systemic immune checkpoint inhibitors, and it shows enhanced efficacy when combined with AXL inhibitors like bemcentinib.
