Overview

The P-Long Protocol is a multi-modal therapeutic regimen developed by Dr. Judson Brandeis for non-surgical penile lengthening and girth enhancement. The protocol integrates monthly intracavernosal injections of autologous platelet-rich plasma (PRP) with mechanical therapies, including daily penile traction using the RestoreX device and vacuum erection device (VED) therapy. The PRP component serves as a biological scaffold that releases concentrated growth factors—such as VEGF, PDGF, and TGF-beta—to promote angiogenesis and tissue regeneration within the corpora cavernosa. These biological effects are synergistically combined with mechanical stress from traction and vacuum devices to induce tissue expansion and cellular remodeling. The protocol also includes oral nitric oxide-boosting supplements (L-citrulline) to support vascular health and tissue growth. It was evaluated in an IRB-approved proof-of-concept clinical study for healthy men seeking penile enhancement.
