Overview

A P-selectin blocking monoclonal antibody is a biologic therapeutic designed to inhibit P-selectin (CD62P), a cell adhesion molecule expressed on the surface of activated vascular endothelial cells and platelets. In sickle cell disease (SCD), P-selectin plays a pivotal role in the adhesion of sickle erythrocytes and leukocytes to the vascular endothelium, a process that triggers vaso-occlusive crises (VOCs) and chronic inflammation. By blocking these adhesive interactions, these antibodies reduce the frequency of VOCs and improve microvascular blood flow. Crizanlizumab (Adakveo) is the first-in-class monoclonal antibody approved for this indication. In preclinical research, such as the study of rilzabrutinib in Townes SCD mice, P-selectin blocking antibodies (often research-grade clones like RB40.34) are frequently used as positive controls to demonstrate the impact of adhesion blockade on microvascular stasis and inflammatory markers.
