P021 (also known as P21 or Peptide 021) is a synthetic, adamantane-conjugated tetrapeptide derived from amino acid residues 148–151 of ciliary neurotrophic factor (CNTF). Developed by Phanes Biotech, P021 is a small-molecule neurotrophic factor mimetic designed for the treatment of Alzheimer's disease and other neurodegenerative disorders. It is engineered to be blood-brain barrier (BBB) permeable and suitable for oral administration. P021 acts by competitively inhibiting the binding of leukemia inhibitory factor (LIF) to its receptor and upregulating the expression and activity of brain-derived neurotrophic factor (BDNF). This dual mechanism promotes neurogenesis and enhances neuronal plasticity. Additionally, P021 inhibits glycogen synthase kinase-3β (GSK3β), which is associated with reduced tau hyperphosphorylation and neuritic damage. The compound aims to provide the neuroprotective and regenerative benefits of CNTF without the peripheral adverse effects associated with the native protein.
