Overview

P140K MGMT-IL13 CAR-T is an investigational autologous chimeric antigen receptor (CAR) T-cell therapy developed by City of Hope for the treatment of high-grade gliomas, including glioblastoma multiforme (GBM). The T cells are dual-engineered to express a CAR targeting the interleukin-13 receptor alpha 2 (IL13Rα2), a tumor-associated antigen frequently overexpressed in GBM, and a mutant form of the O6-methylguanine-DNA methyltransferase (MGMT) enzyme known as P140K. The P140K mutation renders the T cells resistant to the cytotoxic effects of alkylating agents like temozolomide (TMZ) when administered with O6-benzylguanine (O6-BG). This "chemo-protection" strategy allows for the concurrent administration of standard-of-care chemotherapy and CAR-T therapy, aiming to achieve synergistic anti-tumor activity while maintaining the persistence and function of the engineered T cells within the immunosuppressive tumor microenvironment.
