P19 (also known as peptide 19) is a lead cyclic peptide candidate developed by AdvanceCOR for the treatment of Graves' disease and associated Graves' orbitopathy. It is part of a series of cyclic peptides, which also includes peptide 836, derived from the leucine-rich repeat domain of the thyrotropin receptor (TSHR). These peptides function as antigen-specific immunotherapies designed to restore immune tolerance rather than acting as direct receptor antagonists. By modulating the pathogenic activation of the TSH receptor by autoantibodies (TRAbs), P19 has demonstrated the potential in preclinical models to reduce thyroid dysfunction, lower TSH receptor antibody levels, and decrease orbital tissue inflammation and fibrosis. This therapeutic approach aims to provide a more targeted alternative to conventional treatments such as thyrostatic drugs, radioiodine therapy, or thyroidectomy.
