Pablizumab + apatinib is an investigational combination regimen consisting of a programmed cell death protein 1 (PD-1) inhibitor and a vascular endothelial growth factor receptor 2 (VEGFR-2) tyrosine kinase inhibitor. This combination is primarily being evaluated in clinical trials as a neoadjuvant therapy, often in conjunction with chemotherapy, for patients with resectable stage IIA–IIIA non-small cell lung cancer (NSCLC) who do not harbor EGFR mutations or ALK translocations. Apatinib is an orally bioavailable small molecule that inhibits tumor angiogenesis by selectively blocking VEGFR-2, while pablizumab is a monoclonal antibody designed to inhibit the PD-1 checkpoint, thereby enhancing the immune system's anti-tumor response. The synergy between anti-angiogenic therapy and immune checkpoint inhibition is intended to improve pathological complete response rates and overall surgical outcomes in the neoadjuvant setting.
