Overview

PAC-113 is a 12-amino-acid antimicrobial peptide derived from the human saliva protein histatin 5. It is an amphipathic, histidine-rich peptide developed as an antifungal agent for the treatment of oral candidiasis, particularly in immunocompromised patients such as those with HIV or salivary dysfunction[1][2][4][5]. The drug acts by interacting with fungal cell membranes to alter their permeability, leading to cytoplasmic leakage and cell death. Additionally, it interacts with fungal mitochondria to stimulate production of reactive oxygen species that further contribute to fungal cell destruction[1][2][5]. This mechanism is unique among histatin-derived peptides and provides activity against both growing cells and biofilms. Clinical trials have shown that PAC-113 is safe and effective for oral candidiasis therapy in HIV patients; however, its efficacy may be reduced under high-salt conditions[4]. The drug has been formulated as a sugar-free mouthrinse for ease of administration and prolonged therapeutic effect[5].
