Drug intelligence / Profile preview
Paclitaxel is a microtubule-stabilizing small molecule traditionally used as a chemotherapeutic agent, here utilized in a drug-coated balloon (DCB) formulation for the treatment of peripheral arterial occlusive disease (PAOD). In the THUNDER trial, conducted by University Hospital Tuebingen, paclitaxel was applied to the surface of an angioplasty balloon to provide localized, short-term delivery to the vessel wall during dilation of the superficial femoral or popliteal arteries. This mechanism inhibits the proliferation and migration of vascular smooth muscle cells, which are primary drivers of neointimal hyperplasia and restenosis following balloon angioplasty. The THUNDER trial was a landmark study demonstrating that this local delivery significantly reduces late lumen loss and the need for target lesion revascularization compared to standard uncoated balloons, with benefits maintained over long-term follow-up.
