Overview

This combination chemotherapy regimen, developed and investigated by Seoul St. Mary's Hospital, consists of intraperitoneal (IP) paclitaxel administered alongside systemic XELOX (capecitabine and oxaliplatin). It is specifically designed as a first-line treatment for patients with advanced gastric cancer complicated by peritoneal metastasis. The rationale for IP administration is to exploit the peritoneal-plasma barrier, allowing for significantly higher concentrations of paclitaxel within the peritoneal cavity to directly target metastatic nodules while minimizing systemic side effects. Paclitaxel functions as a microtubule stabilizer, oxaliplatin as a platinum-based DNA cross-linking agent, and capecitabine as a prodrug of 5-fluorouracil that inhibits thymidylate synthase.
