This neoadjuvant chemotherapy (NAC) regimen consists of a combination of the taxane-based drug paclitaxel and the platinum-based agent carboplatin. Specifically studied by researchers at Banaras Hindu University, the protocol involves a dose-dense weekly administration of paclitaxel (60 mg/m²) and carboplatin (AUC-2) for six cycles. Paclitaxel acts as a microtubule stabilizer, binding to tubulin to prevent the disassembly of mitotic spindles, thereby arresting the cell cycle and inducing apoptosis. Carboplatin functions by forming reactive platinum complexes that create inter- and intra-strand DNA cross-links, inhibiting DNA replication and transcription. This combination is utilized as a neoadjuvant treatment for locally advanced uterine cervical carcinoma (FIGO stages IIB-IVA) to reduce tumor burden prior to definitive concurrent chemoradiotherapy.
