Overview

This chemotherapy regimen, evaluated in the Eastern Cooperative Oncology Group (ECOG) Phase II trial E2399, was developed for organ preservation in patients with resectable stage III or IV squamous cell carcinoma of the larynx or oropharynx. The protocol consists of induction chemotherapy using paclitaxel and carboplatin, followed by concurrent weekly paclitaxel and radiotherapy. Paclitaxel is a taxane that stabilizes microtubules, thereby inhibiting cell division and inducing apoptosis. Carboplatin is a platinum-based agent that forms DNA cross-links, interfering with DNA repair and synthesis. The regimen aims to achieve high rates of organ preservation while maintaining survival outcomes comparable to surgery in locally advanced head and neck cancers.
