Overview

This combination chemotherapy regimen, consisting of paclitaxel, carboplatin, and fluorouracil (5-FU), is utilized as a neoadjuvant (preoperative) treatment in conjunction with radiation therapy for patients with locally advanced esophageal and gastroesophageal junction (GEJ) cancers. The regimen was evaluated in clinical trials conducted by the Alliance for Clinical Trials in Oncology. Paclitaxel acts as a microtubule stabilizer by binding to tubulin, carboplatin functions as a DNA-damaging platinum agent that induces cross-linking, and fluorouracil serves as an antimetabolite that inhibits thymidylate synthase. Together, these agents work synergistically to reduce tumor burden and improve the feasibility of surgical resection (esophagectomy).
