A-Fudan University-breast cancer refers to Arm A of a Phase Ib/II Bayesian adaptive design umbrella study (FINEST/FASCINATE-N) conducted by Fudan University. This neoadjuvant regimen is specifically designed for patients with HER2-positive breast cancer. It consists of a chemotherapy backbone of paclitaxel (a microtubule-stabilizing agent) and carboplatin (a DNA-alkylating platinum agent), combined with dual HER2-targeted monoclonal antibodies: trastuzumab (Herceptin) and pertuzumab (Perjeta). Trastuzumab binds to domain IV of the HER2 extracellular domain to inhibit ligand-independent signaling and induce antibody-dependent cellular cytotoxicity (ADCC), while pertuzumab binds to domain II to prevent HER2 dimerization with other HER family members. This combination provides a comprehensive blockade of the HER2 pathway alongside synergistic cytotoxic effects to maximize pathological complete response (pCR) rates in the neoadjuvant setting.
