Overview

paclitaxel + cisplatin is a combination chemotherapy regimen being investigated by the Mayo Clinic for the prevention of peritoneal recurrence in patients with high-risk gastric or gastroesophageal junction (GEJ) adenocarcinoma. The regimen is administered via prophylactic hyperthermic intraperitoneal chemotherapy (p-HIPEC), a procedure where heated chemotherapy (approximately 41-42°C) is infused directly into the abdominal cavity for 90 minutes following cytoreductive surgery. Paclitaxel, a taxane, acts by binding to and stabilizing microtubules, which prevents their disassembly and halts cell division in the G2/M phase. Cisplatin, a platinum-based agent, functions by forming covalent DNA cross-links that inhibit DNA replication and transcription, ultimately leading to cell death. This specific application aims to eliminate microscopic tumor cells that may be shed during surgery, thereby reducing the high rate of peritoneal recurrence typically seen in these cancers.
