Overview

This combination chemotherapy regimen, consisting of high-dose paclitaxel followed by the ICE protocol (ifosfamide, carboplatin, and etoposide), was investigated by the Herbert Irving Comprehensive Cancer Center. It is primarily indicated for the treatment of lung cancer in the context of tandem autologous peripheral blood stem cell transplantation (PBSCT). The regimen works through multiple mechanisms: paclitaxel stabilizes microtubules to inhibit cell division, ifosfamide and carboplatin induce DNA damage through alkylation and cross-linking, and etoposide inhibits topoisomerase II to prevent DNA ligation. This intensive approach aims to achieve maximal cytoreduction in patients with advanced or high-risk lung cancer prior to stem cell rescue.
