This investigational combination chemotherapy regimen, evaluated by Seoul National University Bundang Hospital, is designed for the treatment of stage IV gastric cancer with peritoneal carcinomatosis. The regimen, known as IPLUS, consists of intraperitoneal (IP) paclitaxel combined with systemic mFOLFOX6 (modified oxaliplatin, fluorouracil, and leucovorin). Paclitaxel is a small molecule taxane that stabilizes microtubules, thereby inhibiting cell division. By administering paclitaxel intraperitoneally, the treatment achieves high local concentrations directly within the peritoneal cavity to target metastatic disease, while the systemic mFOLFOX6 component—comprising the platinum-based alkylating agent oxaliplatin, the thymidylate synthase inhibitor fluorouracil, and the biochemical modulator leucovorin—addresses systemic disease. This dual-delivery approach aims to improve outcomes in patients with advanced gastric adenocarcinoma and peritoneal metastasis.
