paclitaxel liposome + S-1 is a combination chemotherapy regimen being evaluated by Fudan University for the first-line treatment of advanced metastatic pancreatic cancer. The regimen consists of a liposomal formulation of paclitaxel, a taxane that stabilizes microtubules to inhibit cell division, and S-1, an oral fluoropyrimidine. S-1 is a multi-component drug containing tegafur (a prodrug of 5-fluorouracil), gimeracil (a dihydropyrimidine dehydrogenase inhibitor), and oteracil (which reduces gastrointestinal toxicity). The liposomal delivery system for paclitaxel is designed to improve the drug's pharmacokinetic profile, enhance tumor penetration, and potentially reduce the incidence of hypersensitivity reactions and other toxicities compared to conventional solvent-based paclitaxel. This specific combination is currently being studied in the Phase 4 CSPAC-21 trial (NCT04217096) to assess its efficacy and safety in patients who have not previously received palliative chemotherapy for metastatic disease.
