This combination therapy regimen consists of paclitaxel poliglumex (PPX, also known as OPAXIO or CT-2103) and cisplatin, administered concurrently with radiation therapy. Developed by the Brown University Oncology Group (BrUOG), the regimen is used for the neoadjuvant treatment of esophageal and gastroesophageal junction (GEJ) cancers. Paclitaxel poliglumex is a polymer-drug conjugate where paclitaxel is linked to a biodegradable poly-L-glutamic acid polymer, which improves water solubility and allows for preferential accumulation in tumor tissue via leaky vasculature. Once internalized, active paclitaxel is released to inhibit microtubule disassembly. Cisplatin, a platinum-based chemotherapeutic, induces DNA cross-linking. Both agents serve as radiation sensitizers, and Phase II clinical data (BrUOG-E-215) indicated a significant pathologic complete response rate with improved tolerability compared to standard fluorouracil-based regimens.
