Paclitaxel poliglumex (PPX) plus temozolomide is an investigational combination therapy studied primarily for the treatment of high-grade gliomas, including glioblastoma. Paclitaxel poliglumex is a macromolecular conjugate that links paclitaxel to poly-L-glutamic acid, designed to enhance tumor targeting and act as a potent radiation sensitizer. Temozolomide is an oral alkylating agent that induces DNA damage by methylating guanine residues, leading to cell death. The combination was evaluated with concurrent radiation therapy in clinical trials for newly diagnosed high-grade gliomas. However, the regimen was associated with significant grade 4 hematologic toxicity (notably myelosuppression), which limited its safety and further development as a combined regimen[1][2][5]. Despite this toxicity, some studies reported favorable progression-free and overall survival outcomes compared to standard therapies.
