Overview
Paclitaxel poliglumex + transdermal estradiol is a combination therapy investigated for the treatment of advanced prostate cancer. Paclitaxel poliglumex (PPX, formerly Xyotax) is a macromolecular polymer-drug conjugate consisting of the taxane paclitaxel covalently linked to a poly-L-glutamic acid carrier. This design is intended to improve the therapeutic index of paclitaxel by preferentially accumulating in tumor tissues through the enhanced permeability and retention (EPR) effect, thereby reducing systemic exposure and toxicity. Transdermal estradiol is a hormonal therapy delivered via a patch, designed to suppress the hypothalamic-pituitary-gonadal axis and potentially exert direct cytotoxic effects on prostate cancer cells. This specific combination was evaluated by the OHSU Knight Cancer Institute in a Phase II clinical trial for patients with metastatic castration-resistant (androgen-independent) prostate cancer who had previously received docetaxel chemotherapy. The trial found that the regimen did not demonstrate significant clinical activity in this pre-treated patient population.
