PACOX is a combination therapeutic regimen investigated for the treatment of advanced hepatocellular carcinoma (HCC). It consists of **P**EG-BCT-100 (pegzilarginase), **A** (capecitabine), and **COX** (oxaliplatin). Pegzilarginase is a pegylated recombinant human arginase 1 that depletes systemic arginine, a strategy targeting arginine-auxotrophic tumors like HCC that lack argininosuccinate synthetase (ASS). Capecitabine is an orally administered fluoropyrimidine carbamate that is enzymatically converted to 5-fluorouracil, which inhibits thymidylate synthase and interferes with DNA synthesis. Oxaliplatin is a platinum-based chemotherapy agent that forms intra- and inter-strand DNA cross-links, inhibiting DNA replication and transcription. The combination aims to leverage the metabolic vulnerability of HCC cells while providing synergistic cytotoxic activity.
