Overview

**palbociclib + famotidine** is a combination of two pharmaceuticals: palbociclib, a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor indicated for the treatment of advanced hormone receptor–positive, HER2-negative breast cancer, and famotidine, a histamine-2 receptor antagonist used to reduce gastric acid secretion and treat gastrointestinal conditions such as gastroesophageal reflux disease (GERD) and peptic ulcer. Palbociclib exerts its antineoplastic activity by selectively inhibiting CDK4 and CDK6, preventing cell cycle progression from G1 to S phase, thus inhibiting tumor cell proliferation. Famotidine acts by blocking histamine H2 receptors on gastric parietal cells, lowering gastric acid secretion. Combination therapy may be considered for patients on palbociclib who require gastric acid suppression, but pharmacokinetic research shows that, unlike proton pump inhibitors, famotidine does not significantly affect the exposure or efficacy of palbociclib when dosed in a staggered manner[3].
