This therapeutic regimen combines pamidronate, a second-generation bisphosphonate, with vitamin D and calcium carbonate to prevent rapid bone loss and fractures in patients following kidney or heart transplantation. Pamidronate acts by inhibiting osteoclast-mediated bone resorption, specifically by targeting farnesyl pyrophosphate synthase (FPPS) in the mevalonate pathway, which leads to osteoclast apoptosis and reduced bone turnover. The regimen was evaluated in a prospective, open-label, single-arm study (NCT00302627) sponsored by Providence Health & Services. In this study, patients received intravenous pamidronate (60-90 mg) every six months for two years, supplemented by daily oral vitamin D and calcium carbonate. This combination therapy is designed to counteract the accelerated bone loss often triggered by the high-dose glucocorticoids and other immunosuppressants required post-transplantation.
