Drug intelligence / Profile preview
Pamidronatdinatrium (pamidronate disodium) is a nitrogen-containing, second-generation bisphosphonate small molecule drug that acts as a potent inhibitor of osteoclast-mediated bone resorption. It is widely approved under the brand name Aredia (developed by Novartis) for the treatment of moderate to severe hypercalcemia of malignancy, Paget's disease of bone, osteolytic bone metastases of breast cancer, and osteolytic lesions of multiple myeloma. Mechanistically, pamidronate disodium binds to hydroxyapatite crystals in the bone matrix and is internalized by osteoclasts, where it inhibits the enzyme farnesyl pyrophosphate synthase (FDPS) in the mevalonate pathway, preventing protein prenylation and ultimately inducing osteoclast apoptosis. Additionally, it is being investigated by Aarhus University in a Phase 2 clinical trial for the treatment of chronic non-bacterial osteomyelitis (CNO), a sterile inflammatory bone disorder, where it may also exert immunomodulatory effects by reducing Vγ9Vδ2 T-cell receptor gene expression.
