Pancrelipase is a porcine-derived pancreatic enzyme replacement therapy (PERT) consisting of a mixture of digestive enzymes, including lipases, proteases, and amylases. It is primarily indicated for the treatment of exocrine pancreatic insufficiency (EPI) associated with conditions such as cystic fibrosis, chronic pancreatitis, pancreatectomy, and pancreatic cancer. By replacing the endogenous enzymes that the pancreas fails to secrete, pancrelipase facilitates the digestion of fats, proteins, and carbohydrates in the small intestine, thereby improving nutrient absorption and preventing steatorrhea and malnutrition. Various formulations exist, typically as delayed-release capsules containing enteric-coated microspheres or microtablets to protect the enzymes from gastric acid degradation, though immediate-release tablets are also available for specific clinical needs.
